Scholarship Application
Guidelines for the Hart and Soul of Our Community Scholarship
In order to apply for this scholarship, the candidate must:
1. Be a graduating high school senior in Brazoria County, TX planning to further their education at a community college, university, or trade school.
2. Provide one of the following:
a. Proof of participation their school’s Community Service (Service Cord) Program with a brief teacher statement stating such along with the number of clocked community service hours
OR
b. A letter of recommendation from an individual verifying their involvement within the community. This letter should be provided by a person who has overseen the student’s participation in community events. It may not come from a family member.
3. Complete the scholarship application (page 1).
4. Provide a copy of your school transcript
5. Submit a 300–500 word essay answering the following question:
“Tell us about the most important service contribution you made to your community, and who inspired you to make that contribution?”

**Optional: Attach no more than 3 recommendation letters
**Family members of Mr. or Mrs. Hart are not eligible to apply
**Incomplete applications will not be considered
